Thank you for completing your online paperwork for your upcoming appointment with Dr. Sameer Mathur. We are honored you have chosen us as your health care provider. Our goal is to provide the highest quality of care for all of our patients in a timely and respectful manner.

Please make sure you have completed all of the forms listed below in their entirety. Once complete please submit each form online. Please note that if this paperwork is not completed your appointment may be delayed.

• New Patient Symptoms and Medical History Form
• New Patient Demographics
• Financial Policies and Procedures
• Acknowledgement of the Use and Disclosure of Heath Information for Treatment or Health Care Options

On the day of your appointment, please bring with you:

• Government issued photo ID
• Insurance card
• Copies of operation records, medical records, X-rays, MRI’s, CT scans, bone scans, and ultrasounds.
• If you have seen a physical therapist, please bring a progress letter from the therapist.
• If you have had surgery elsewhere, please bring a copy of your operation report if that is available.
• Your health insurance information.
• Your claim number if it is a workers compensation injury.

We ask that you allow plenty of time to get to the office for your appointment. You may be asked to reschedule your appointment if you are more than 15 minutes late. We will strive to stay on time. From time to time, a patient emergency arises and we may be running late for your visit. You will have the option to reschedule or stay to be seen and we will keep you informed of how long of a delay you may experience.
